**Job Title: Registered Nurse (RN) Program Manager**

**Salary Range:** $98,000 - $131,000 per year

**Job Description & Responsibilities**

We are seeking a dedicated and experienced Registered Nurse (RN) Program Manager to oversee the daily operations of our specialized health programs. This role involves integrating evidence-based care practices into protocols, policies, and continuous quality improvement initiatives. The ideal candidate will supervise the team to ensure patients meet eligibility requirements and coordinate with health plans to implement appropriate services.

**Key Responsibilities:**

- Manage day-to-day activities of specialized programs and/or products, establishing workflows and protocols, and recommending improvements as needed.
- Act as a liaison for care management teams to ensure programs meet expected outcomes and implement necessary changes.
- Lead and participate in internal and external clinical care conferences, serving as a resource for care managers and promoting staff understanding of tele-management processes.
- Educate and improve patient knowledge of chronic disease management, self-care, and identification of health status changes through individualized education and interventions.
- Review patient records for readmissions during active home care episodes, leading root cause analysis and developing quality improvement measures.
- Analyze productivity reports, implement corrective measures, and conduct routine team audits.
- Assist staff and senior leadership with patient navigation, metric reporting, and clinical data analysis.
- Perform managerial duties, including staff training, performance evaluations, and recommendations for hiring, promotions, and terminations.
- For specialized products, work with partners/vendors to set up devices, troubleshoot technical issues, and participate in technology implementation across the organization.

**Qualifications:**

- **Licenses and Certifications:**
- Current registration to practice as a Registered Professional Nurse in New York State.
- Valid driver's license may be required.
- Care Management or Case Management certification within one year of job entry date.

- **Education:**

- Relevant degree needed for professional licensure.
- Master's Degree in a healthcare-related field preferred.

- **Work Experience:**

- Minimum of three years of clinical experience.
- Experience in case management, administration, or discharge planning in a hospital setting preferred.
- Training in population care coordination preferred.
- Exceptional customer service skills and ability to engage clinical counterparts in collaborative discussions.
- Strong follow-up skills and ability to manage multiple priorities.
- Proficiency in Microsoft Office Suite.
- Knowledge of value-based care models and managed care preferred.
- Hospice or palliative care experience preferred.
